Lead, Patient Partnership

Job Summary:

Reporting to the Director, Patient Partnership & Experience, the Lead, Patient Partnership will collaborate with patients, families, caregivers, clinicians, leaders, and staff to advance patient- and family-centred care across Windsor Regional Hospital and Erie Shores HealthCare. The role is responsible for fostering meaningful patient engagement and partnership in the design, delivery, evaluation, and continuous improvement of healthcare services, programs, and initiatives.

The Lead, Patient Partnership will champion the patient voice by developing and implementing strategies, frameworks, and opportunities that empower patients and families to actively contribute to decision-making processes at both the operational and organizational levels. Working in partnership with interdisciplinary teams, the incumbent will support quality improvement, patient safety, equity, and experience initiatives, ensuring that patient perspectives inform planning, policy development, service redesign, and organizational priorities.

Duties and Responsibilities:

• Adhere to policies and procedures established for the assigned department while supporting the Mission, Vision and Values of Windsor-Regional Hospital and Erie Shores HealthCare.

• Re-imagine and establish a dynamic PFAC (Patient and Family Advisory Committee) focusing on a patients care journey.

• Incorporate the voice of patients, families, and caregivers into shaping how care is designed and delivered through PFAC related projects, work plans and strategic planning.

• Work with hospital leaders to implement infrastructure and processes necessary for advisor engagement; prepare staff and clinicians to work with patient and family advisors, and assists with the recruitment, training and ongoing support to advisors as needed.

• Develop and train advisors and staff through PFAC and partnership orientation.

• Oversee advisor activities, providing mentoring and coaching, and facilitating the ongoing engagement of advisors.

• Support programs in the implementation of strategies to improve patient satisfaction.

• Research and recommend methods/strategies to improve patient satisfaction.

• Create, implement and monitor mechanisms for ongoing input from patients and family advisors into policy, procedure and operations.

• Participate in education related to strategic plan initiatives as appropriate.

• Engage in rounding to support patient/family needs and liaise with experience team to address feedback/concerns in real time.

• Perform any other relevant duties as assigned.

Qualifications:

• Undergraduate degree in Social Sciences, Nursing, Adult Education, or related field of study required;

• Bachelor’s degree in Social Work considered an asset;

• Maintaining and promoting a safe and clean working environment for all employees, students, visitors, patients/clients, family members and physicians and fulfilling the duties of workers under the Occupational Health and Safety Act.

• Reporting any observed risks to the appropriate authority whose actions or behaviours towards clients are unsafe or unprofessional.
